And hey, if you're a fallen paladin, why not go all the way and become a Blackguard? I'm sure Redcloak could summon up an Evil Outsider for him to make contact with.

You can't tell me that a Blackguard O-Chul wouldn't be a badass addition to Team Evil.

A twist I truly believe will never happen, mainly because they are both dead now and I see no way it could bring something to the plot in the future: Miko being Shojo's daughter.

V is killed by Xykon, ends up in the LN afterlife (yes, I know, lets not start that discussion) and encounters Miko there.

Despite the initial inevitable antagonism, they eventually decide to put aside their differences, seeing as they are dead and all and there's not much point to fighting.

Miko is still in denial over having fallen from grace, and feels that she was wronged. Vaarsuvius however realizes and accepts that s/he has done horrible things, intoxicated by the power as well as the freedom from consequences s/he thought she had due to the false "control" of the souls.

Durkon is about to raise V, who swears that s/he is going to make up for the misdeeds of her life and atone, in the hopes of avoiding an eventual return to the LN afterlife (which I view as being boring and bureaucratic).

Miko is inspired by this, and finally accepts that the only way to regain her state of grace is to acknowledge her faults. This allows her to be given a second chance, and she returns to the world as a ghost to aid the Order of the Stick against Xykon.

The MitD is never revealed, but just fades away into the background of the story.

Soon Kim was actually a black Guard of enormous power, and the order of the rifts were all evil. The Snarl is in fact an immensely powerful being of good. Xykon is really an inevitable trying to free it, and the Dark One is infact one of the twelve god's.

Shojo neglected to mention he's got another ninety levels in assorted classes, and couldn't be resurected because he never died.
