Lepidrihh are dreaded parasites of jungles and rainforests. They appear to be large butterflies, but their burrowing teeth suggest otherwise. All wise travelers avoid them, and those who become their victims are treated or outcast. Lepidrihh are hermaphrodites, and immediately seek to reproduce after they emerge.

Combat:
Lepidrihh have only one tactic: burrow in the nearest living thing and nest. If they are threatened and can find no suitable living creature, they flee.
Burrow: If a lepidrihh hits a Small or larger living creature with its bite attack, it starts to burrow. This takes 1 round, during which it cannot take any other action. This is otherwise treated as grappling. If it is not detached, it deals an additional 1d4 damage and is inside the body. It cannot be removed without cutting the body open or a DC 25 Heal check. As soon as it is inside, it begins to nest.
Nest: Once burrowed inside a living creature of suitable size, a lepidrihh immediately begins to nest. This process takes 1 minute, after which the lepidrihh immediately dies. A DC 19 Fortitude save prevents the eggs from surviving. Either way, the lepidrihh sustains the victim for 15 days without need for food, drink, or sleep. If the eggs were successfully laid, they hatch after this period, dealing 1d4 Strength and 1d4 Constitution damage that cannot be healed for 2 weeks without a heal spell. During this time, the victim feels as if they have “butterflies in their stomach”. After the 2 weeks, the larvae form chrysalises, and the sickness goes away. However, 2 weeks after that, 1d6 new lepidrihh emerge and eat their way out, causing 1d10 damage per lepidrihh. The new lepidrihh immediately begin mating, then seek out new victims. Any disease removing spells or effects kill the eggs/larvae/chrysalises. Creatures immune to disease do not suffer any negative effects from a lepidrihh’s Nest ability.

CR 7, based on the DC of the Heal check, CR 6 based off of the BAB. I'd go with 6.
