So, which forms are the best ones for polymorphing oneself into at each level?

At level 7 I would vote for Treant and Hydra. The first gives you incredible natural armor and 29 str, while the other one gives you 7 attacks per full attack and a str of 19.

Also what spells are good for buffing before polymorphing?

It depends on what you want to accomplish. In Core, those are pretty good. If you happen to be a Sorc (or otherwise have insane Cha), Cloakers kick ass. Generic Pouncers & Swallow Wholers are also pretty good.

The Troll isn't bad either; decent Str, decent NA, Rend, etc. Annis Hag is also quite good. Phthisic [XPH] dealing Intelligence-damage is really nice too.
