Threads not being marked as read

I'm sure it has something to do with the cache. I know I get a problem that threads will "reset" to not being read if I close out of the window and then come back within a few minutes, but this doesn't sound like the same thing.

I'm gonna agree with Squat. Your browser may be pulling a cached version of the forum index that has all those threads marked as unread. Check your cache settings. Find out how often your browser checks the cached version of the page against the current. Since the pages are dynamically generated, though, you could still have a problem.
