I remember somebody mentioning once an item called the phylactery of faithfulness. It supposedly tells you before you do something whether or not it will make you fall. Can anybody tell me where I can find this?

As a bonus, it's a small box you strap to your forehead. If that doesn't say cool I don't know what does.

No the real bonus is that if a Cleric of Lolth uses it they insta-fall just for using it if you go by her fluff...

Would that apply to Paladins (the CE variant in this case)?

Not to Paladins. It applies to clerics of Lolth because part of her fluff is that she likes to keep the Drow guessing and never knowing quite what she wants them to do. Using a magic item to defeat that part of her dogma is like spitting in her face.
