what were your favorite solve or die puzzles in game? and did your party die or survive?

personally mine was during a crawl through a literal dungeon. there was a puzzle involving placing orbs in slots in a correct order. if the pattern wasnt correct we had to start again. not only did the correct pattern open the doors it also stopped the ceiling from decending on us and crushing us to death. the dm also had us on a real timer. we had a 5 minute limit (real time) to go over clues and solve puzzle. we died but was fun as hell.
