Make sure you'll be happy with the name change, they don't like you doing it too often (it confuses the Mod's Borg-like information storage systems).

I believe there is a one-year period in-between name changes that they ask you to respect.

Been able to since a long while, as far as I know. Since sometime before 04-01-2009, in fact.

Since always. As long as it's forum appropriate, within length limits and not taken, you can do it after 6 months.

Make sure you use the proper format requested in the OP of the thread linked above (they won't change it unless you do).
