The PCs start off as passengers in an airship that gets hijacked by sky pirates. Roll initiative.

The party members happen to be on the same train to Sharn/Korth/Flamekeep/Fairhaven/<insert metropolis here>. A passing airship drops a squad of warforged shock troopers onto the train. The train starts going out of control. Roll initiative.

Of course, that can be added to either one of the above two starts...

The characters are in the bar on an airship that gets hijacked...
The characters are in the bar on a lightning rail that gets attacked...

Of course, starting at 4th level means you can just hand-wave the idea that the characters have been adventuring together for awhile.

I'm about to start one myself. So far, we've got one who joined up with a caravan as a guard, another who's traveling with that caravan on the way to investigate a large disturbance, and two who are being driven out of another country and who join up with the caravan as it travels across the desert. They all end up traveling with the one investigating, and when they get to where he's going, they'll end up rolling for initiative at some point.
