Most guides have good forms from lvl 7 as that is the lowest lvl you'd be to actually cast it.
Some critters get it as a spell-like however, and so they could get lower lvl forms.
Now, what forms with 6hd are good for combat or utility?

Consider looking at the Druid Handbook (http://brilliantgameologists.com/boards/index.php?topic=1354.0) for ideas.

Our Druid loves the Fleshraker Dinosaur (4HD) for attacks.

There doens't seem to be a size restriction on polymorph (http://www.d20srd.org/srd/spells/polymorph.htm) so I agree that a Six-Headed Hydra (http://www.d20srd.org/srd/monsters/hydra.htm) looks promising.

There's some decent forms. Troll is a fair 6HD Brawler, for example. It has hands and it speaks so spellcasting should be possible too. Desmodu Guard Bat [MMII] is pretty nice. Least Dusk Giant [HoH] is also fair. Just plain Tiger is a nice charger/grappler. And yeah, Fleshraker isn't bad either. Bog Imp [HoH] has a 60' burrow speed.

I also recall there being a feat that lets you have the supernatural abilities as well. There is one for the psionic version, im just wondering if the polymorph version was a homebrew version of that one. Anyone sure?
