Resembling nothing more than a massive heart hovering in the air, a red dawn's heart is in fact a malevolent and dangerous inhabitant of the Far Realm. Occasionally released into the Material Plane by a freak accident of creation, Red dawn's hearts are menacing predators with no place in the natural order. They seek to drain blood from those around them, to sate some unknowable thirst. They have a diameter of about three feet, and weigh approximately ninety pounds, most of which is pure muscle. They do not speak, but seem to understand Undercommon.

Combat
Blood Drain (Ex): A red dawn's heart deals 1d4 points of Constitution damage with a successful grapple check. Blood drained in this way is stored in a reservoir inside the red dawn heart's body and can be consumed by the creature (see Sanguine Consumption). A red dawn's heart can hold up to 10 points of Constitution in its reservoir at any one time. A randomly encountered red dawn's heart has a reservoir holding 1d8 + 2 points of Constitution.
Improved Grab (Ex): To use this ability, a red dawn's heart must hit with a pseudopod attack. It can then attempt to start a grapple as a free action without provoking an attack of opportunity. If it wins the grapple check, it establishes a hold and can use its blood drain special attack.
Venomous Ichor (Ex): Whenever it is dealt damage by a slashing or piercing weapon, a red dawn's heart sprays forth some of the vile fluid that passes for blood in its body. It exposes the attacker to a contact poison that has an initial and secondary damage of 1d4 Str. A Fortitude save (DC 17) allows resistance to the poison. Creatures attacking the red dawn's heart with reach weapons do not endanger themselves in this way. The save DC is Constitution-based.
Powerful Build (Ex): The way a red dawn's heart's body is built allows it to act in many ways as if it were one size category larger. Whenever it is subject to a size modifier or special size modifier for an opposed check (such as during grapple checks, bull rush attempts, and trip attempts), the red dawn's heart is treated as one size larger is doing so is advantageous. However, its space and reach remain those of a creature of its actual size. The benefits of this racial trait stack with the effects of powers, abilities, and spells that change the red dawn's heart's size category.
Sanguine Consumption (Su): A red dawn's heart can consume the blood it has drained from targets to fuel its own natural healing. Consuming a point of Constitution heals the creature of 5 points of damage. Alternately, it can consume 3 points of Constitution to grant itself the effects of a haste spell, as though cast by a wizard of the Red Dawn's Heart's HD.
Telltale Heart (Ex): A red dawn's heart takes a -20 penalty to Move Silently checks, as the constant, thunderous pulsing of its whole body runs contrary to stealth.

I love Far Realm creatures.

You might want to mention an upper limit to the amount of Constitution it can store, or set a time limit in which the blood curdles, dissipates, or otherwise becomes useless.

Maybe even a dice roll to determine how much blood the creature contains when a battle begins with it?

"Telltale Heart" probably needs to just be a -20 or -30 penalty. As it stands it could be interprited that everyone in the world can hear it, even if they are in a deep cave on the other side of the planet. Which actually would be a pretty cool effect for... say a Great Old One or something now that I think of it.


EDIT: Or save some time for everyone, and say that the base DC before distance, intervening obstacles and etc are taken into account is always -5. (I still think that would be an interesting effect for something on a really high level).

Perhaps a nifty ability for it would be to creep inside a creature of Huge size or larger and implant itself, replacing the creature's heart (and reanimating it if it was dead), making it an undead creature with low Wis, high Cha (to reflect the madness) and bestowing its own abilities on it?
