(Most) reach weapons leave a 'hole' in the PCs' threatened squares at 5' around her.

If a character is wielding a two-handed reach weapon, and wearing a spiked gauntlet, does she threaten adjacent squares - or does the two-handed weapon preoccupy both hands? Does it make a difference whether she's just used the two-handed reach weapon to make attacks? For example, could she full attack with the reach weapon and then make AoOs with both it and the gauntlets? Could she mix both in one full attack?

What about reach weapon + armor spiked instead?

Also, under SRD: Armor, it says armor spikes can be used as a light off-hand weapon. Can you TWF with a two-handed weapon + armor spikes? If so, does the former still deal 1+1/2 Str damage?

I don't know the exact answer as by the rules, but from a logical standpoint, you could swing with your reach weapon at the man in front of you, and then take a hand off it and backhand the man at your side with the gauntlet. All you'd risk is easier disarming of your reach weapon in the turns before you can put your hand back on it. Does that sound right?

If a character is wielding a two-handed reach weapon, and wearing a spiked gauntlet, does she threaten adjacent squares - or does the two-handed weapon preoccupy both hands? The two-hander occupies both hands. Letting go of it with one (or both, but you don't want that) hand is a free action, as is grabbing it again when holding it in one hand (sage ruling).

You can only use free actions on your own turn. So no, you don't threaten with the glove when holding a two-hander with both hands.

What about reach weapon + armor spiked instead? You threaten and can claim AoO with both.

We have a Raptoran Stormtalon in our party, and this is exactly what she does.
Spiked armour and reach weapon.
Once she qualifies to use her foot talons as secondary attacks (not just as extras in a diving full attack sequence), she can ditch the spiked armour.
