My brother is looking to play a Cleric of Pestilence using a heavy flail reminiscent of WHFB plague monks (but not the class monk)

Pestilence domain is a given. Possibly a magical flail with a limited Stinking Cloud or similar effect (custom magic item)

What other feats/affects would add to this concept?

And is there anything out there that would fit for warpstone poison/taint/mutation?

If you have the Book of Vile Darkness, I suggest giving it a read. There should be at least something in there for you.

The Smoking weapon enhancement is basically stinking cloud on a weapon, so no need for homebrew there. (It's also greatly underpriced). You *could* use the Taint rules from Heroes of Horror, but that's a terrible idea. They're very, very bad. There are various feats that might be useful, like Aberration Heritage or the Deformity feats, to represent mutation.

The Cancer Mage PrC from BoVD would be relevant. Unfortunately, it's either broken (via a loop it enables where you get infinite strength) or rather weak (if you ban that loop).

It does seem underpriced...but it could be interpreted that the wielder is still penalized for the 50% concealment when trying to target others and possibly even line of sight issues if by one-half concealment they mean 50% miss or total concealment.

But, the whole 'not affects wielder in any way' seems to negate that....

Soooo ya, 50% miss and no line of sight or AoO, and smoking cloud inside area seems way to good for +1 weapon

Especially one that can be put on a thrown weapon/returning to move that smoke around to nauseate

So, Skaven? I think a kobold would be the closest equivalent. Also:

Explosive: Whenever you hit with this weapon, everyone within 5 ft of your target (including the target, and potentially you) takes an additional 2d4 points of fire damage (Save for half). Could easily be houseruled to be acid, or disease. Obviously you'd want to put it on a reach weapon, or get Fire resistance. Complete Warrior pg 134.
Enfeebling weapon enhancement: On a critical hit, deals 1d6+2 points of temporary Str damage. +1 bonus, Book of Exalted Deeds pg 113.
Lethe’s Lash: A spellcaster hit by this whip loses the highest level spell or spell slot they can cast. 23,301 gp, Planar Handbook pg 80.
Chain of Obeisance: +2 Unholy Spiked Chain. If you successfully Pin your enemy in a Grapple, they must make a Will Save or be dominated as per Dominate Monster. Requires True Believer feat and 13 HD. 35,600 gp, Complete Divine pg 94.
Mohrg’s Tongue undead graft: Gives you a secondary touch attack that paralyzes enemies. 24,000 gp, Libris Mortis pg 80.
Mummified Hand undead graft: Gives you a Slam attack, which you can deliver Mommy Rot with a limited number of times per day. 16,200 gp, Libris Mortis pg 80.
Paralyzing weapon enhancement: Any creature hit must Save or be paralyzed. They get a new save every round. +2 bonus, Book of Exalted Deeds pg 113.
Binder: Naberius vestige gives you Faster Healing ability, which lets you heal 1 point of ability damage every round. Useful if you want to go into Canger Mage, use Vile feats (very potent), or abuse drugs (http://www.giantitp.com/forums/showthread.php?t=148101). Also grants amazing party face tricks. Astaroth vestige grants a pretty excellent disease based breath weapon. Fort Save or you're Nauseated and then Sickened, and even if you pass you're still Sickened. Also grants at will Suggestion. Zceryll grants the psuedonatural template (diseased/aberration appearance - gives you Spell Resistance and various other perks) and unlimited Summon Monster (which in turn grants you access to unlimited spells, since many summoned monsters can cast).
