The title says the question, I'm sure there was a belt or similar that gave that template whilst worn.

Please and thank you to anyone who can tell me the items name and which book I can find it in.

Dark template is granted by the Collar of Umbral Metamorphosis (Tome of Magic p. 156). The standard action version lasts up to 10 minutes per day (divided up as you see fit) and costs 10800 GP. The continuous-use version costs 22000 GP.

As far as I know, there isn't an item that offers the Shadow template. Be careful which version you're using, too. The Manual of the Planes version was updated in the 3.5 update book (LA +3), but the version printed in Lords of Madness (LA +2) supercedes the MotP version.
