I wonder if a spell could be made that would permanently kill a target? I do not mean destroy it in corporeal form, but also disintegrate its soul, so that clone/lichdom would be rendered useless? Maybe something that would "attach" itself to a soul directly and eat it away after one failed save?

Anyone good here with creating epic spells of this type?

Try this (http://www.d20srd.org/srd/spells/trapTheSoul.htm). Then you just have to hide the gem.

Hmmm. So all it would need for it to be effective is to make it ignore SR, contingencies and for it to have a very high save?
How about destroying that soul?

Can see a few ways of doing this. A lot of them involve trapping the soul. Trap the soul in a gem, hide the gem. Create an enchantment that traps the soul in the plane of the dead. Bind the soul to something in the land of the dead. F

A more creative method might be to forcibly reincarnate the motherf***er so they have to find what is probably a baby sparrow somewhere and kill it to get the soul back for resurrection/lichdom.

Alternatively, come up with a rite to transform them into an elemental, then kill them. Their soul and body become one and thereafter merge with their plane. Variant on both the destroying and the trapping design; trap the soul in the body so that even when the body dies it can't escape.

All really depends how souls and resurrection works in your campaign.

Necrotic Termination, from Libris Mortis. Basically does exactly this on a failed save.
