I have an item written down on my character sheet: Amulet of Change, and was wondering if anyone knew what book it was from. It allows the wearer to cast polymorph self 1/day and to return to natural form at will. Any help would be appreciated!

Phylactery of Change. 11200 GP, A&EG p. 135. All-day polymorph, up to 7HD. However, it's a 3.0 sourcebook and the polymorph spell has been through about half-a-dozen rewrites since then, and may require a little tweaking or hand-waving.
