Does temporary intelligence (gained through items or the like) give you Extra skill ranks?

No. Only permanent bonuses give additional skill ranks, and even they do not give them retroactively - only future levelups get the benefit.

Normally the description for the augmentation method will say if it affects skill points, a headband will not, and it is described in its description, however a tome will, and it is stated there as well, and as it already has been said, it is not retroactive.

This character has Psychic Reformation manifested on him. The player erases all the skill points, and starts to put the new ones in, but no one at the table remembers at what level the tome was applied.

How is the number of skill points this character has then determined?

So, say a character is level 15 or so, and has used an Int tome at some point.

This character has Psychic Reformation manifested on him. The player erases all the skill points, and starts to put the new ones in, but no one at the table remembers at what level the tome was applied.

How is the number of skill points this character has then determined?

There is no net change in skill points. Psychic Reformation undoes skill points and feats, freeing them up to be reassigned. The 'undoing' is done in reverse order of how levels are gained -- skill points and feats associated with newer levels are removed first, but reassigned last.

Strictly speaking, no relevelling is involved. Also, by RAW, each individual level has to be tracked separately, 'though this shouldn't really matter as the XP cost per level is relatively trivial anyway.

Forgetting when the INT tome was applied may make things a bit tricky, but you can always calculate the total amount of skill points by comparing skill ranks and whether the skills are cross-class or not. Note that this assumes that all skill points are spent on each level. (but are there any players who don't spend all available skill points on level-up?)

Multi-classing will probably complicate matters, but the calculations are still doable.

Sorry, when I said "The player erases all the skill points" I was implying that the information of the number of skill points the character had before the Psychic Reformation has been lost irrevocably by accident.

In that case it's up to the player and the DM to negotiate on how many skill points the player should get. Best guess should be enough. Note though that a single INT tome won't have made a difference unless the player was on an odd number of INT points when they used it.
