Are there any magic items that allow a PC to get into melee quick, allowing a full attack on the first turn? I guess that would mean swift activation.

Failing that, are there any other good magic items for a barbarian character to use at the start of combat?

Most people do this with the Spirit Lion Totem ACF in Complete Champion which grants the Pounce special ability (Full Attack at the end of a charge).

The main problem is that many teleport effects end your round after you hop. The few exceptions (e.g. Anklets of Translocation) have a very short range. There is a psionic ability called Dimension Hop that allows you to swift action teleport, can be augmented beyond it's standard 10' range, and doesn't interfere with follow on activities.

Belt of Battle, MIC 73. Swift activation 3 charges/day, use 1 for a move action, 2 for a standard action, or 3 for a full round action.

A wand of Lion's Pounce or a dorje of Psionic Lion's Pounce, or a dorje of Dimension Hop as mentioned above.

A master level shadow hands (the item from ToB that grants you a maneuver) will give you Shadow Blink, a 1/encounter 50 ft teleport as a swift action for 45,000 gp.
