I am playing a Factotum in an upcoming game and have been looking for ways of dealing with the random person quickly and quietly. For the point of roleplay and because it is just so damn awesome, I have been looking for rules on using a Garrote but have come up mostly empty. The rules I have found make it less useful than just stabbing the person in the back with a knife despite the downsides.

Are there any decent rules for using a Garrote that is better than just "I attack the flat-footed dude"?

As they have not been reprinted since, they are valid for a 3.5 campaign.

The Dragon Compendium also had rules for garrote attacks; pages 112-114.

Dragon Compendium has the 3.5 update of them. They're really quite poor, though. A poisoned dagger would be far more effective.
