I would have thought the obvious would be "Tiamatist".

Races of the Dragon also has a Draconic glossary, which lists "tiathar" as the Draconic for "spawn of Tiamat".

But most game materials referencing followers of Tiamat refer to them as "followers of Tiamat".

If they are particularly devoted to the blue and green heads aspect, of course.

(The red ones are Tiamaytoes. Some of them prefer to be called Tiamahtoes.)

The problem, thinking on it more and looking at lists of Earth religions, is that very few are named after their principle deity or prophet. The only major ones that are I can think of are Christians, Buddhists, and Jehovah's Witnesses. ("Mohammedan" is deprecated and considered offensive.)

All the others tend to be prosaically descriptive terms. "Jew" derives from Hebrew for "People of Judea", "Baha'i" derives from the Arabic for "glory" or "splendour", "Muslim" is just Arabic for "one who has faith", and so on.

So, if you really want to be realistic, you would come up with the Draconic for "seeker of power" or "follower of the five-headed path" or "breeder" or something else that describes what the philosophy of Tiamat seeks to accomplish, and call them that.

Those exclusivity devoted to the green side are called Tiamatillos (http://en.wikipedia.org/wiki/Tomatillo).
Those in a (losing) conflict with the red aspect followers are Fried Green Tiamaytoes.

So, for Cyric, it's Cyricist.

And for Tiamat, it's Tiamatan.

We are ignoring the abortion that is 4E Forgotten Realms.

I think they are reffered to as Thralls, as in "Thralls of Tiamat". That might be a specific title for her Clerics though, rather than a general title for her worshippers.
