[3.5]Charger build looking for advice

So I was thinking about making a charger and thought along the lines Barbarian/Bear Warrior/Frenzied Berserker. Then I thought even further into maybe minotaur or Ogre for large size and increases to Str/Con but as I write this I guess transforming into a Bear would negate the need of large sizes from minotaur or ogre.

1. What would be a good class build out (without the use of Tome of Battle)be?
2. What about race selection?
3. Any advice on flaws or traits would be helpful as I am completely new to them and have never used them despite owning the books...

Edit: Sorry forgot to mention that we would be starting at level 21 and preferably no multi-class penalties would be nice.

1. Maybe something along the lines of barbarian2/fighter2/fist of the forest3/bear warrior 5/FB rest. Pugilist variant fighter would be best.
2. Avoid Str or Con penalty, LA and RHD.
3. Traits are more flavour. Flaws with least impact for this include Meager Fortitude (you have all good fort classes, high Con and Great Fortitude feat), Vulnerable, Shaky and perhaps Murky-Eyed. You don't necessarily need flaws to pull it off, but they help.
