I've seen a couple high-level ones out there, but never any epic. Does anyone know if Wizards has ever printed a module that starts at level 21+?

There's one module in Dungeon Magazine (though I forget which one, exactly - it was something about killing a god/whatever in a hidden demiplane before somebody can activate a god-killing weapon).

The Throne of Bloodstone was a module for "100th-level" characters. It was AD&D though and doesn't translate well (if at all) to 3.5.

Does anyone know if Wizards has ever printed a module that starts at level 21+?Inside the "Epic Level Handbook" is the adventure "Kerleth's Tower", which is stated to be suitable for a group of epic characters of levels 21-23, and by the end should advance an epic character by one level.

Several Planescape adventures could probably make fine epic adventures. I'm thinking of things like Squaring the Circle and Dead Gods.

Dungeon Magazine 123, 135, 140, 150 have an adventure for epic level characters.

Bastion of Broken Souls and An Icy Heart are Wotc adventures for level 20 characters. An Icy Heart is free on the Wotc website.

The Drow Wars is a 3rd party campaign that starts at level 1 and goes up to level 30.

Shackled City, Age of Worms, and Savage Tide are campaigns from Dungeon that I believe get into epic levels at the end.

Rappan Athuk, Ruins of the Dragon Lord and the World's Largest Dungeon are 3rd party campaigns that also go up into epic levels.

Elder Evils and the Epic Level Handbook have some ideas for epic level campaigns.

Dungeon 93 - Storm Lord's Keep, for lv21 party.
Dungeon 123 - Quicksilver Hourglass, for lv30 party.
Oerth Journal 22 - Until the Starbreak, for lv21+ parties (scaleable as the final boss is a cr36 xexical!) :smalleek:
Oerth Journal 24 - Sundered Spark, for lv22+ parties

All that I am aware of.
