Recording forms for Wildshape

Anyone have any good suggestions for formats to write wildshape'd forms in?

Copy the monster format, as it's pretty condensed, and leave out the CR and other things you don't need. Basically:

Size
Initiative
Speed
AC
Attack
Space/Reach
Special Attacks
Special Qualities
Modified Saves
Modified physical abilities only
Any skills that have changes

You might want to look at the NPC format in the DMGII, IIRC it has a nicely condensed way of handling things like Wildshape and Rage.

A few of them, but also in a nearby chapter (after or before, don't quite remember). They actually give you a list of sections that go into the format at the end of the chapter.
