where could I find all of the rules pertaining to difficult terrain - i.e., its effects on movement, tumbling, AoO, etc.

going along with that, if a character tried to tumble through the threatened squares of a large creature with a reach weapon, would they have to make a new tumble check for each square moved through, at successively higher difficulty? the tumble skill requires that you check for each opponent passed, and each opponent passed after the first is +2 DC - but the wording seems to imply that you are only in a threatened square once.

essentially, how easy is it for someone to tumble up to an enlarged knight with a reach weapon, the bulwark of defense ability, and the stand still feat.

Unless Bulwark of Defense and Stand Still specifically mention anything about it, then it is just a Tumble DC 15+2 for each enemy past the first. If the Tumble check passes, the tumbler provokes zero attacks of opportunity for the movement in which they tumble.

how fast would they move though? difficult terrain already reduces speed to 1/2, as does tumbling - would their speed be 1/4? and if they failed would they provoke AoO for each square they exited (setting up an epic channel the storm)?

In the Player's Handbook pg 163, Table 9-4 explains the movement penalties, and pg 164 goes into greater detail.

Looks like difficult terrain ought to increase the check to 20 at least and speed should be 1/4 because there's no issue of in what order 2 speed halving effects apply.

If they fail the check, they provoke AoOs as they would if they were doing any other sort of movement action that would normally provoke AoOs. So if that would be one for each square if they were not tumbling, then it would be the same number if they fail the check.
