Every Lich, though formidable, pales in power compared to the Demilich. There most likely isn't a Vampire that does not aspire to become a Vampire Lord.

Are there any other templates that can only be applied to a creature with a specific template, and are an explicit "better" version?

Does Evolved Undead count?

It can only be applied to undead, and is defiantly an upgrade.

In the 3rd party book Immortals Handbook - Epic Bestiary there's the Akalich template, which can only be applied to a Demilich who has existed for millenia and is so strong that they have lost their body entirely. It has a LA of +100 I think.

That book is insane.
