Hi there, I was wondering if anyone knew a word or term in the same vein as gender or marital status that pertains to alive or dead?

Not exactly, but something like vital status might work...

Well, there's no common place to put it on a form, because people are usually assumed to be alive when they fill out a form.
However, if a person is dead, I think they usually just write deceased, and maybe the date and/or year of their death after it.
Example:
Gene Eric (deceased, 2003).
Or, they put one of those little crosses and the date after their name. But that's probably not used very much any more.
Widow for women and widower for men. The two words indicate that their spouse is dead. Example: A widow has a dead husband (or wife if legal).
