There was a module that I remember seeing, I'm pretty sure it was for 3.5. It was essentially a giant dungeon module that began in a cemetary. I believe the last level involved a fight with Orcus? It was a huge dungeon, with a wide range of encounter levels, depending on the layer. Anyone know what I'm talking about?

I think we might need a bit more than including a cemitary and maybe Orcus.

Sounds like Headless, an adventure that appeared in Dungeon #89.

Sounds like the 1e/2e adventure H4, Throne of Bloodstone. The first (that I know of) module intended for play by characters up to 100th level (the listed module levels were 25-100, with a set of notes for how to scale it at 25, 50, 75, and 100).

Featured such sights as the City of 100 Liches and the Tarrasque, and a chance to meet both Tiamat and St. Cuthbert.

Sounds like Rappan Athuk (http://rpg.drivethrustuff.com/product/12529/Rappan-Athuk-Reloaded?it=1) by Necromancer Games.
