Undead Leadership and Undead Cohort

A question on Undead Leadership. Does my Undead Cohort they have to be undead with Int? Creatures from the Monster Manuals? or Undead as Class levels such as in Libris Mortis?

Your Undead Cohort has to have a listed LA, just like a normal cohort. The exception to that rule is only for followers, and only for the undead listed in the table in Libris Mortis.
