I've taken a little bit of inspiration for this rule from Tengu_temp's first post in this (http://www.giantitp.com/forums/showthread.php?p=13775402#post13775402) thread. My idea is a lot riskier than his but I like mine more. It does however assume use of the Tome of Battle so the exact execution of the idea might not be the same for every group. So with no further ado, death or glory attacks.

Death or Glory Attack
A wounded tiger is the most dangerous of all, for he has nothing to lose for lashing out with all his rage and fury.
When beaten within an inch of his life a warrior* can draw upon a great adrenaline-fueled burst of strength enough to perform superhuman feats of skill. In game terms when ever a martial initiator is reduced to 1/4 or less of his normal hit point maximum he can charge and execute a bull rush maneuver while full attacking against a single target. Now unlike normal full attacks the warrior uses his full BAB for all attacks in that round. In addition the warrior can make two martial strikes he knows and has readied, though one of them has to be unexpended. As a rule of thumb standard action strikes take up one normal attack per full attack action. Also, all ability score damage and penalties from maneuvers are maximized, if they don't deal flat ability damage already, and all bonus damage, if any, is empowered (http://www.d20srd.org/srd/feats.htm#empowerSpell) even if it's not randomized.

Unlike a standard bull rush you knock an enemy back by 5 ft. plus 5 ft. per ever ten feet you charged. You also make a succesful bull rush if you roll your enemy's Strength score or higher on a d20+your Strength-10+BAB.

*By warrior I mean anyone with at least medium BAB and maneuvers.
