I forget exactly what source the dream dimensions are listed in 3 and 3.5 (and P if you got it), I think it may have been a dungeon mag?

Either Manual of the Planes or Planar Handbook. I always get the two mixed up.

Manual of Planes has it. But I think most of the crunchy stuff is actually in Heroes of Horror.

MotP. Plane is called Dream or Dreamscape. Also the source of the Lucid Dreaming skill.

The plane of Dal'Quor is also a dream dimension that exists in the Eberron setting. Information about it can be found in the Eberron Campaign Setting book and a bit more information about its denizens can be found in Secrets of Sarlona and Magic of Eberron.

There's not much, but in addition to being in MotP and HoH, there is a slight mention of a "Dreamscape" in the Epic Level Handbook. It's only a small thing, but there's an Epic monster that can weave nightmares, travel to and from the lands of dreams, and I think can send others there as well. There is also an Epic Level spell that takes you there, kind of like an Astral Projection/Shadow Walk effect.
