Campaign Book Format?

So yeah, I've never actually seen an honest-to-goodness guidebook with a campaign in it, and I've been thinking of a bunch of potentially wonderful campaign ideas (Personal Opinion).

Could anyone sort of tell me how those books are formatted, or give me an example, so I'd know how to go about making one?

Your best bet would be to take a well-known setting and mimic its formatting(I suggest Eberron). In general, it reflects the PHB.

General Overview
Races
Classes
Character Options(Feats/Spells)
Prestige Classes
The world(Geography and such)
Organizations
Monsters are either before or after organizations
Sample Adventure(not always included, but it is a nice touch)
