OK..

I can never get my head round this one. Do you get the option of making a two handed attack even if you dont have an extra attack due to BAB. Or does 2 weapon fighting only apply to iterative attacks? If you do two-weapon fight, what kind of action is it?

Attacks gained from Two-Weapon Fighting and it's subsequent feats are not affected by iterative attacks. You merely gain additional attacks based on whichever feat that you have (+1 TWF, +2 ITWF, +3GTWF). Your iterative attacks are influenced by your TWF, specifically the penalty to your attack bonus.

Two-weapon fighting (normally) only works with full attacks (which are full round actions). Iterative attacks from main hand are separate from off-hand attack(s), and (except for the attack penalty) work exactly like you weren't using TWF. Off-hand attacks are extra.

Iteratives only work with your mainhand weapon. The offhand weapon gets its attacks from the TWF feat chain.

Can you 2 weapon fight without having a feat or special ability that specifically allows for it?

Can you 2 weapon fight without having a feat or special ability that specifically allows for it?Yes. You probably won't be able to hit anything, but you can try.

Yes... at huge penalties, and you would not have access to improved/greater two weapon fighting. Only 1 attack.

Okay, so you can ALWAYS attack with two weapons (as a full round action). If you attack as a standard action, it is with one attack, even if you have two weapons. As you gain more BAB, then you can attack more with your main weapon. However, it requires feats to be able to attack more than once with your second weapon.

Scout is a level 5 Ranger with 14 Strength. He chose the two handed path and so he has Two-Weapon Fighting (http://www.dandwiki.com/wiki/SRD:Two-Weapon_Fighting) feat. His attack order goes like this, +5/+5.(5 BAB+2STR-2Fighting with two weapons=5).

After fighting a battle against an Orc, Scout can now reach level 6. His BAB goes up by 1 and he also gains the feat Improved Two Weapon Fighting (http://www.dandwiki.com/wiki/SRD:Improved_Two-Weapon_Fighting). His attack order goes as follows. +6/+6/+1/+1.

If Scout instead took a level of Barbarian( thus not having the Improved two Weapon Fighting feat) his attack order would look like this. +6/+6/+1.

So the answer your question's are: Yes, you may attack with a weapon in your other hand(albeit at a very high penalty with out the feat) even if you dont have the BAB to gain a second attack with your main weapon. When you have taken the correct feats, then you can make more than one attack with your second weapon. Any attack that has more than one attack involved( unless said attack is caused by the first attack) is a full round action. Hydras are an exception cause of their heads.

Scout is a level 5 Ranger with 14 Strength.Took me a while to parse that. "What, is he talking about swift hunter?"

He chose the two handed path and so he has Two-Weapon Fighting (http://www.d20srd.org/srd/combat/specialAttacks.htm#twoWeaponFighting) feat. His attack order goes like this, +5/+5.(5 BAB+2STR-2Fighting with two weapons=5).…With a light off-hand weapon, otherwise the penalty increases to -4.

Consider a level 20 fighter using a normal full-attack action with a one-handed weapon. Base attack bonus is 20. So the bonuses for each iterative attack would be:

first iterative attack +20 / second iterative attack +15 / third iterative attack +10 / fourth iterative attack +5

Now the same fighter uses a one-handed weapon in his off hand to make a second attack using the standard two-weapon fighting rules:

first iterative attack +14 / first off-hand attack +10 / second iterative attack +9 / third iterative attack +4 / fourth iterative attack -1

Again, the same fighter uses a weapon in his off-hand, but this time it is a light weapon.

first iterative attack +16 / first off-hand attack +12 / second iterative attack +11 / third iterative attack +6 / fourth iterative attack +1

Now if that fighter had the two-weapon fighting feat he could make an off-hand attack with a one-handed weapon as follows:

first iterative attack +16 / first off-hand attack +16 / second iterative attack +11 / third iterative attack +6 / fourth iterative attack +1

Or he could make an off-hand attack with a light weapon as follows:

first iterative attack +18 / first off-hand attack +18 / second iterative attack +13 / third iterative attack +8 / fourth iterative attack +3

With Improved Two weapon fighting and a light weapon he can do this:

first iterative attack +18 / first off-hand attack +18 / second iterative attack +13 / second off-hand attack +13 / third iterative attack +8 / fourth iterative attack +3

With Greater Two weapon fighting and a light weapon he can do this:

first iterative attack +18 / first off-hand attack +18 / second iterative attack +13 / second off-hand attack +13 / third iterative attack +8 / third off-hand attack +8 / fourth iterative attack +3

Now going back to a first level fighter with two weapon fighting and a light weapon he can take his full attack action if just a single +1 attack and now do a first iterative attack -1 / first off-hand attack -1.
