A friend of mine once made a sword with the bonuses of Both Banesteel and Bloodsteel. Is this in a book, or did he just homebrew this?

Separating Double-bladed Sword?
You could probably make the two blades out of different material.

It was a single scimitar. I'll assume he homebrewed it.

The SRD says it all:
If you make a suit of armor or weapon out of more than one special material, you get the benefit of only the most prevalent material. However, you can build a double weapon with each head made of a different special material.

I don't know of any exceptions.

IIRC, Oerthblood and Pure Ore can both be added to any other metal, but those are the only ones I know of like that.

Depends how long ago you mean by "Once made".

The older Metalline property from Underdark allows the weapon to change into any metal. Though only 1 type at a time.

MIC has since updated it to change into only a few standard metals.

There's also Glassteel from the 3.0 FR books. It's a material by itself, but combines the properties of both Mithril and Adamantine.
