Recently I infected one of my players with a disease/curse thingy from a bored demonic humanoid. I decided i should write up what all it did, so here it is. Constructive criticism is wanted here as i haven't finished tier 3.

I recommend removing the alignment change. The skin rotting doesn't fit the general demonic theme this has. Do you know what you are looking for, for stage 3?

Im looking for stage 3 to be a drastic character change, that has just enough power and cool stuff to tempt a character into taking the permanent change. But the changes have to be a luck of the roll type of thing, so that its never a sure thing, and it will keep me from having a whole party that wants to be Tainted.

Then what might work is spell like abilities, supernatural abilities, and various other things demons get. Open up the MM's, and the Fiend Folio 1 is all about demons. Drop the possibility various, even more drastic, physical changes. Try some things that the player can't just pass off as having one odd parent.

Hmm. Various stat changes could be in order, depending on the nature of the change. On your stage 3 table (assuming you are making one) have a few things that are only bonus to one ability score, some things that boost one or two and drop the same amount in others, and a few that are negatives to a single ability score.

I agree with removing the alignment change, at least for neutrals. And it has nothing to do with the fact that I don't think I could play chaotic evil very well.
