How do you do cones that are not aligned with the grid or at a 45° angle, or is there a rule that this can't be done?

Do you trace two lines (as per line atacks) at a 90° and affect anything between those lines?

Basically what I do, depending on the arc (45, 90, 135, 180 degrees ect) of the effect.
Only thing I kept my old protractor from school for :smalltongue:.

Pretty much. Here are some visual aids (http://www.d20pfsrd.com/magic/area-effect-diagrams) to help.

(Though those are for Pathfinder, they're identical to the 3.5 shapes.)

You have what you need to draw those - as you said, trace two lines (as per line attacks) from the chosen corner of the square and affect whatever is between them out to the cone's max range.

If you always (or mostly) use a grid of a given size, you could rather easily make a template out of cardboard, plastic, or the like, for easy reference.

Like the WH40K templates:

Then you can just rule that squares that mostly fall under the template are affected, which should be pretty fast if you use grid and minis.
