This may be obvious but my mind refuses to work right now: Are grapple checks subject to concealment (ie 20% or 50% miss chances)?

No, because they're not attacks. Concealment does nothing when you're literally holding the other guy by the throat and punching him in the gut.

Even the initial attempt?

The roll to grab them? Yeah. The rolls to hold on? Nope. Suprisingly sensible for WotC.
