What's that monster?! It's ___!

I need a bit of help finding something. In the picture of it, it's depicted as a creature that's basically just the head of a mind flayer and is either a tiny or a small, but other than that I don't know pretty much anything about it. The description said something about something going bonkers during the changes and caused it to turn into such an odd thing.
Sorry for the lack of detail, that's literally all I can remember about it :smallfrown:

Pretty sure it's a larval mind flayer from what you describe. I believe the stats were in Expanded Psionics Handbook, or in Complete Psionics.
