Is there a good item for changlings (or other disguise-based characters) that lets them modify their clothes to match there appearance? Or even a mundane way to deal with this, short of carrying around a department store of clothes and items?

There's a magical shirt-slot item called shiftweave in either Races of Eberron or MIC that is -exactly- what you're looking for, and it's dirt cheap at like 500gp.

Shiftweave is in MIC. It limits you to 5 different outfits. The standard Hat of Disguise (http://www.d20srd.org/srd/magicItems/wondrousItems.htm#hatofDisguise) can be made to look like anything you want, but at 1800 gp it costs more.

There are other options: The reserve feat Face-Changer (CM) allows you to do Disguise Self at will. Also, the spell Clothier's Closet (MoE) allows you to truly create clothes worth up to 100 gp (meaning they're non-magical and don't go away in antimagic). An eternal wand of Clothier's Closet is useful for any character.

There's also the Glamered enchantment for armor in the DMG that will fit your needs too.
