Hengeyokai Lycanthrope

Would a Hengeyokai Lycanthrope only have the 5 forms or would there be a third hybrid form, half human, quarter hengeyokai animal, quarter lycanthropic animal?

Humanoid, lycanthrope hybrid, hengeyokai animal, and lycanthrope animal. What would be the fifth?

Hengeyokai hybrid would be the 5th.

Edit:
Lycanthropes can also adopt a hybrid shape that combines features of the base creature and the base animal.

The animal form is still a part of the base creature so could one technically also become a half lycanthropic animal, half hengeyokai animal?

Was thinking of doing that with a shapeshift druid and reserve mage so I could be utilizing my spell power at low level while shifted. Do Hengeyokai or Lycantropes loose their (Su) and (Ex) abilities when they shift? AFB atm.
