I was looking at the free sample on wizards.com, and have 3 questions:
1) Can a multiclass barbarian initiate while raging?
2) Similarly, can a multiclass druid initiate while wildshaped?
3) What's the formula for highest level maneuver known? Is it the standard 2*level-1, or is it something else?

1. Yes, although they probably cannot use the diamond mind ones that involve the concentration skill.
2. Yes. The only requirement for a menuvre is that you be able to move.
3. Yes.

Yes, but note that initiator level is equal to your class level plus your levels in initiating PrCs plus half the rest of your levels. So a Fighter 4/Warblade 5/Eternal Blade 2 has IL 2 + 5 + 2 = 9, and can select 5th level maneuvers.
