Can you make an extra attack with haste or the speed property or when frenzy/whirlwind?? Also, does weapon supremacy +5 bonus on attacks apply on AoO???

Nope, 1 attack / round is all you get (you are talking about the alternate fighter ability at lv16 in PHB2, right)?

Don't think so. It's really geared towards iterative attacks only, IIRC.
