Do they turn back to their base humanoid form immediately upon dying, or do they die as a beast?

I believe that since it works as Polymorph, other than as stated, you return to your base form.

it worked as polymorph in 3.0. in 3.5, it is keyed off alternate form (http://www.d20srd.org/srd/classes/druid.htm#wildShape)

but yes, you still change back when killed.

Gotcha, thanks guys. :)

Was just thinking on the potential of a necromancer killing a druid in beast form and raising it into a beast, keeping it permanently risen and rotting in that form.

If it doesn't work like that it's cool, was just wondering whether it was possible. Thanks again.

I suppose the Necromancer could raise the druid as an intelligent undead and have them take Undead Wildshape.

This works. Just as simple as using Create Undead and then Control Undead, right?

Yeah I'd think so.

Yeah I'd think so.

but the creatures from create undead don't retain their class levels, since none of them are templates.

Create undead isn't going to raise the dead druid, it's going to create one of the undead from the list that it says it can create.

Which undead creating spell lets them keep class levels again? I forget..
