Ok, so you've got a creature with a diseased attack (http://www.d20pfsrd.com/bestiary/monster-listings/undead/daughter-of-the-dead). Do you calculate the disease every time the attack hits, or only the first time?

Roll for disease chance each time the PC is hit and makes his save until the creature is dead or the PC fail's his save. Once the disease attack is successful calulate and stop checking for disease chance (you can't get double diseased).

On a side note: How the heck do you resist disease with Charisma? Sweet talk the plague virii into not infecting you? "hey baby...you know I'd love do have you coursing through my veins but I don't want my syphilis finding out I'm two-timing it...how about a rain check?"

"The Save DC is Charisma-based" is a reference to the user of the ability, not its target. It means that if the Daughter's Charisma is increased, her disease will get correspondingly more deadly. (read: difficult to save against.)

Ok...still....does SHE sweet talk the plague virii? Does she hold some kind of plague pep rally and bounce around in a cheerleader outfit before attacking?

Basically, Charisma is used in place of Constitution for many undead, since they lack a constitution score. Generally, poisons and diseases are based on Con - the more vitality a monster possesses, the more toxic and virulent its chemical weapons are. With undead, Charisma is used so that they can be improved (or debuffed, as necessary.)

Charisma represents sense of self moreso than persuasiveness and attractiveness. Having a strong sense of yourself can certainly lead to you being more persuasive, and it often makes you more attractive to many people, but those are secondary effects and not the main focus of the stat.

You think that's bad?

Lots (http://www.d20pfsrd.com/classes/core-classes/monk) of people (http://www.d20pfsrd.com/classes/core-classes/paladin) add Wisdom or Charisma to nonsense like Armor Class. How in the flying f*** does that work? Are you shouting riddles at incoming arrows? Commanding swords to miss you? Doesn't make any damn sense.

Wisdom makes a fair degree of sense, with the proper training. Wisdom is about quick, gut level thinking and perception. Dodging blows by looking where they are headed and not being there makes a good deal of sense.
Charisma to AC generally comes from magical sources and so tends to be more nebulous.

CHA is kinda weird...it's a representation of your force of personality. Gone are the days when it was strictly for physical appearance and eloquence.
It's kinda like STR for mental stats, really. And oracles, bards, and sorcerers really do get a lot of really weird things they can do with the stat.
