I've only seen higher ground mentioned twice in the handbook. Once in a table and once in the mounted combat section. When exactly do you get it?(In the mounted combat section it only mentions you get that bonus it doesn't say that's the only time)

There are a couple of cases aside from being on a horse.

If you are attacking into a trench from outside the trench, you gain higher ground. You also gain higher ground if you are attacking someone on gradual stairs (but not steep stairs).

There are no other cases I've seen that grant the higher ground bonus.

the easy way to discribe it is If you are on higher ground. The PH just gives it in a table with no expansion text.

What about if you're flying down and striking someone below you?

Presumably because you have to pay some attention to staying upright on steep stairs and someone lower than you has an advantage of being able to aim for yer legs (your ballency bits :P) which negates the high ground bonus, just my 2cp.

No idea, but the bonus isn't listed in their description.

Dwarven Defender + Talisman of the Disk (500 GP, MIC) = higher ground. Put the talisman on the Pouncebarian for a Dwarven Sidecar.
