Whoever has the HHH can get stuff out of the HHH. Just like a regular backpack.

So, while someone who is not wearing the bag may retrieve an item from it, they may have to do some serious digging around for it.

How about if my Homunculus or Familiar is stowed in the bag. Can they hand stuff out to me?

And if so does it take me more actions to get the item from them than retrieve it myself?

A Packmate (Magic of Eberron) can. In fact, it's telepathic specifically so it knows what you want and when you want it.
