So if you cast dust of sneezing and choking into the air and it's a 20ft spread, how are you not effected by it.

Throw it into the air above your opponents, instead of above yourself. If you're not in the area of effect then you're not affected by it.

Yeah but it's a handful of dust, it's going to spread as soon you release it meaning you'd be caught in it. Remember it's a cursed item so the intent is someone doesn't know what it is, throws it, and screws themselves.

Teleport Object. Dust is an object; even if not one mass, Teleport Object allows one to move an object "up to 3 cubic feet per level". You can touch the Dust without triggering it; only casting it into the air does that. Teleport it five feet above an opponent's head, and it's thereby cast into the air.

The Necklace of Adaptation (http://www.d20srd.org/srd/magicItems/wondrousItems.htm#necklaceofAdaptation) wraps you in a shell of fresh air. Dust never touches you.

Get Mettle. When you beat the DC 15 save, nothing at all happens.

Have an Unseen Servant, a Summon, or some other disposable minion use the dust (you don't care if it coughs a bit and/or dies).

A 7th level spell would be a pretty high price to pay but it doesn't work on magic items so it won't work anyway.

I knew there was at least one item that had that effect but I couldn't remember what it was.

At the very least I think the cheese of DoCaS is somewhat offset.

I'm not sure that works. The effects of the dust have nothing to do with whether your breath it in or not. If you are in the area of effect and are not immune to stun, you will be stunned even if you make the save. Talisman of Undying Fortutide (MIC) should work, though.

There's another oddity to the RAW of the dust... if you fail both saves, you take the Con damage but you're not subject to the choking/stun at all.

(A few days ago, I was wondering if an Air Goblin with the Breathless (Ex) ability could ignore the dust, and came to the conclusion it couldn't.)

from the SRD:

Those who succeed on either saving throw are nonetheless disabled by choking (treat as stunned) for 5d4 rounds.

If you fail both saving throws, you didn't succeed on either of them
