A Sphere of Annihilation, an Umbral Blot, and putting it in a Bag of Holding and tearing the Bag. These are the ways to 'utterly destroy' something (or someone).

How many other ways are there to 'utterly destroy' something? Bypassing immortality, regeneration, artifact status, even divinity to a point.

As a side, how many ways are there to recover something/one from such methods of destruction?

Using a soul as a spell component annihilates it, as does a barghest devouring it.

Unname can destroy any object, even artifacts, with no chance of failure.
Step 1: Take a level 1 commoner or a house cat or whatever (if it's Int is less than 3 then hit it with Fox's Cunning to give it an Int score of more than 3).
Step 2: Hand it the item to be destroyed.
Step 3: Cast Unname on the creature from step 1.

The creature can theoretically be returned to existence but the items can't be, they are totally obliterated and don't even get saves.

Course for Artifacts only there's Apocalypse from the Sky, which similarly destroys them utterly as it gets used up as a material component.

Casting the following spells on a creature means they can't come back. They're not dead, so they can't be revived (not even through Clone), and their body is gone, meaning there's no real way to get it back:

Flesh to stone, then Rock to Mud, then Purify Food and Drink. Then either drink the water or scatter it.

The only way to get it back is through a Wish.

1) Put an item into a bag of holding, then put a bag of holding into a bag of holding.
2) Bag of devouring is fine too.
3) Give an item to somebody, then planeshift somebody to Ravenloft. Neither an item, nor the peson will be ever heard from again.

Unless they pull a Lord Soth.

1) Put an item into a bag of holding, then put a bag of holding into a bag of holding.
Does nothing at all. You can stack Bag of Holding's to whatever extent you desire with no problem.

3) Give an item to somebody, then planeshift somebody to Ravenloft. Neither an item, nor the peson will be ever heard from again.
Wish can pull the individual out, and the item if it is still on them.

putting it in a Bag of Holding and tearing the Bag

This doesnt destroy it, only makes it lost.

Necrotic Termination (Libris Mortis 69) is, well, exactly what it says on the tin. Even Wish and Miracle cannot bring the victim back to life, so. Costs 1000 XP, though.

So for living creatures, there's that.
