What's the best way to increase your hit points? Obviously there's more con, toughness, improved toughness, etc.

What are some good spells or magic items that increase hit points?

Temporary hit points are generally where it's at. The Psionic Power Vigor is quite sexy, at 5thp per ML.

Fey Mysteries Initiate can grant int to hp... this may even work with undead characters.

Speaking of which, a Necropolitan character created near an Desecrated Evil Altar by a Dread Necromancer or UA variant Necromancer gets D12+4 HP/HD, which might be a serious improvement for some characters.

Venerable Dragonwrought Kobolds qualify for Epic Toughness right away, which grants +30HP.

There's no known cheese for gaining epic toughness aside from dragons is there.

You do need to be a dragon (aged old or older) to bypass the normal restrictions on getting that feat.

The Vitality Belt soulmeld can give you quite a bundle-- level times essentia investment. With the Expanded Soulmeld feat, that caps out at a hundred extra HP at 20th level-- more if you take levels of Incarnate. Necrocarnum Vestments give you 3*essentia HP, and just shaping Totem Avatar gives you +level. Go Incarnate 20, take Expanded Soulmeld Capacity twice, and you're talking over 160 extra health, all the time.

Interestingly, of that is real HP, not bonus HP.

Dusk Giant cheese might work, I'd have to check what happens if you gain feats with your extra HD and then lose the extra HD. Might need Dark Chaos Shuffle in there too.

"Good spells" is a pretty broad category, and the answer will depend on your circumstances. If you've got Minor Shapeshift [Reserve], any polymorph spell of 4th level or higher ready to cast will give you +HD of temporary hit points with a swift action — round after round. If you've got sneak attack, Vampiric Touch is a very good spell to have in a wand with Use Magic Device, because the spell gives you temporary hit points equal to the total damage your attack deals, including Craven, Knowledge Devotion, and whatever other damage bonuses you have.

If you can turn/rebuke undead you can use Divine Vigor. +2 temp HP per character level and +10 movement speed for 1 minute per charisma modifier. Uses 1 turning/rebuking attempt.

An important point: For short-duration buffs, temporary HP are better than Con increases, but for long-duration buffs, Con increases are better. If you use a Con-increasing effect, take some damage, and the Con increase wears off, you're still damaged (and might even be dying or dead, if it was enough damage). On the flip side, though, if the Con boost lasts a long time, you can heal damage, but you can't heal back temporary HP (though you might be able to gain them again the same way you did in the first place).
