Do "sanctum spell" and "brew potion" work with each other to craft 4th level potions that would work outside of the sanctum?

IIRC yeah they work that way, but no you aren't supposed to use them that way.

In a low optimisation low level game? Sure go ahead.
In a high-op high-ish level game? Not a good idea.

It's a slippery slope, so beware.

No, this won't work. Metamagic feats can only make spells in items a higher level than normal, not lower.

Using metamagic feats, a caster can place spells in items at a higher level than normal.

Metamagic feats don't actually change the level of the spell, normally. That text is referring to applying metamagic with slot-level adjustments, not applying metamagic that changes the level of the spell itself. Sanctum Spell technically doesn't change the slot that the spell uses. It changes its spell level while leaving it in the slot of its original level.

Some metamagic feats do change the spell level - Sanctum being one, and Heighten being the other. But regardless, the rules text specifically says "level" not "slot."

If you really need a polymorph potion, use the Master Alchemist PrC, as it allows you to create potions above 3rd level. I use it for "Mana Potions" in my campaign setting.
