The sparring dummy of the master from the Arms and Equipment Guide allows you to take a 10' step any time you would normally be allowed a 5' step (anyone who isn't a monk must use UMD to benefit from it). I know there are sources of a second 5' step out there (and thus, a second 10' step), but I can't seem to remember what they are.

I think that Oriental Adventures has a rule that lets you get one from a very high Tumble check.

This thread covers it. (http://www.giantitp.com/forums/showthread.php?t=153629)

It's not a 5-ft. step, but the Wolf Pack Tactics stance allows to move 5 feet every time you successfully strike someone.

Press the Advantage stance, meanwhile, does exactly what the thread is asking.

Net and Trident Style (http://www.giantitp.com/forums/showpost.php?p=16236646&postcount=2) can also be used to gain another 5' step.

Anklets of translocation let you teleport 10ft as a swift action.

For Pathfinder, the Step Up (http://www.d20pfsrd.com/feats/combat-feats/step-up-combat---final) and Following Step (http://www.d20pfsrd.com/feats/combat-feats/following-step-combat) feats allow you to take a 5' step whenever an adjacent foe makes one to get away from you.

The Sidestep feat (MH) allows you take take a 5ft step after an AoO 1/round, which does not count towards your normal 5ft step. The Tactical Soldier PrC gets it as a bonus feat (2nd level), without needing to meet the usual prereqs.
