Is all of Thistletop map, the jungle area supposed to be difficult terrain and squeezing for Medium Creatures?

Description is in Page 42 Area C2: Thistle Tunnels.

Yes, the whole thistle area is like that, except maybe C3 which is a larger "chamber".
